Welcome to The Wellbeing Agent. This document outlines the terms and conditions for services offered by The Wellbeing Agent, a UK-based software provider of nutritional therapy, mental health coaching, and continuous glucose monitoring (CGM) plans. Please review these terms carefully before engaging with our services. By scheduling an appointment or using any services, you agree to be bound by these terms.

Definitions  

– “The Wellbeing Agent” refers to the business entity providing nutritional and mental health services, including but not limited to CGM plans.
– “You,” “Client,” or “User” refers to the individual engaging with or purchasing services from The Wellbeing Agent.
– “We,” “Us,” or “Our” refers to The Wellbeing Agent, including any freelancers or independent contractors working under its direction.

 Scope of Practice

The Wellbeing Agent provides evidence-based nutritional consultancy, mental health coaching, and CGM plans to support brain health and overall wellbeing. Services are tailored to individual needs and may include the use of tools like continuous glucose monitoring to improve health outcomes.
Please note:
– Non-medical Advice: Our services are not a substitute for professional medical advice, diagnosis, or treatment. We encourage clients to consult their healthcare provider for any health concerns, particularly regarding serious or unexplained symptoms.
– Freelancers and Contractors: The Wellbeing Agent may engage independent contractors to assist in service delivery. These contractors are bound by these terms but cannot offer services independently without our prior approval.

Client Responsibilities

As a client of The Wellbeing Agent, you are responsible for the following:
– Commitment and Participation: Your success depends on active engagement, following our program guidance, and ongoing communication.
– Health Conditions: You are responsible for disclosing all relevant health conditions that may impact the advice or plans we provide.
– Medical Conditions: We do not diagnose or treat medical conditions. Should you seek a diagnosis from a licensed dietitian or medical professional, The Wellbeing Agent is not responsible for their findings or actions.

Freelancers & Independent Contractors 

Freelancers or independent contractors may assist in providing services, particularly in administering CGM plans or offering additional nutrition advice through third-party platforms like ‘That Clean Life’ or ‘Cronometer’. Contractors will only have limited access to your health information and cannot act on behalf of The Wellbeing Agent without express authorization. Disputes arising from independent contractor services must be addressed directly with The Wellbeing Agent, though we assume no liability for their actions.

Consultations

Our services include the following types of consultations:
– Initial Consultation: Lasts up to 90 minutes, where we will discuss your health goals and create a plan of action.
– Follow-Up Sessions: These monthly check-ins last up to 60 minutes.
– Short Check-Ins: 30-minute sessions for ongoing support.
All consultations are conducted online via ‘Practice Better’, ‘Google Meet’, ‘Zoom’, or other telehealth platforms, and may also be conducted in-person at a rented facility in London, UK.

Booking Appointments  

Appointments can be scheduled through the following:
– Practice Better platform for free chat consultations.
– Email, phone, or telehealth for all other appointments.
Bookings are subject to availability and are confirmed upon receipt of payment.

Payments  

– Full payment or the first installment of a payment plan is required before your initial consultation. Payments are accepted via bank transfer.
– Non-adherence to agreed payment plans may result in termination of services or legal action.

Cancellations, No Shows, & Late Arrivals 

– Cancellations: You may cancel or reschedule your appointment without penalty up to 48 hours in advance.
– Late Cancellations/No Shows: Appointments canceled or rescheduled less than 48 hours before the scheduled time will incur a 50% cancellation charge.
– Late Arrival: If you arrive late, the session will still end at the originally scheduled time.

Refunds

Refunds are not offered for missed appointments, late arrivals, or cancellations made less than 48 hours before the appointment. Exceptions may be made in extenuating circumstances at the discretion of The Wellbeing Agent.

Privacy & Confidentiality

Your privacy is a top priority.
– Client Privacy: All personal and health information is treated confidentially and used solely for the purpose of providing nutrition and mental health services.
– Data Security: We use secure systems, including ‘Practice Better’, to manage and store client data in line with industry standards.
– Third Parties: Your information will not be shared with third parties without your explicit consent, except when necessary for the delivery of services (e.g., working with freelancers).

Working with Multiple Practitioners

Where necessary, and with your consent, The Wellbeing Agent may consult other practitioners involved in your care to ensure coordinated service. All shared information will align with your preferences and privacy rights.

Liability Disclaimer

Results vary among individuals, and success depends on your commitment and adherence to the program. The Wellbeing Agent makes no guarantees regarding specific health outcomes. Results are dependent on individual circumstances and adherence to the program. We do not assume responsibility for actions, advice, or recommendations provided by freelancers or independent contractors, including but not limited to nutritional personalization, behavior change coaching, mental health coaching, mindset coaching, meal plans, supplement recommendations, functional medicine protocols, or lab testing interpretations.
